If you would like to make a contribution to Big Lake Hospice in memory of someone who has died, you may mail it to us directly or make your contribution electronically by clicking on Online Donation below.
When making a memorial contribution, tell us:
• Name of Deceased
• Your Name
• Your Address
• Your Daytime Phone
• Name & Address of Deceased Person’s Family Member (so that we can notify them of the gift you make in memory of their loved one)

Big Lake Hospice will acknowledge receipt of you gift within two business weeks after receiving it; the family member of the deceased, who you have asked to be notified of your gift, will be contacted at the same time. The amount of your gift will NOT be shared with the family member.
